The number of people living with chronic illness is growing rapidly
The number of people in England, living with one or more clinical long-term conditions, is going to grow substantially over the next 7 to 15 years.
The NHS and Local Authorities are not coping now, so will be overwhelmed if the model of care is not changed quite radically to emphasise prevention rather than cure.
