Objectives of the diagnostic
Use available data to scale the forward demand challenge from the growth of chronic disease in the target system population and assess the impact on local providers.
Undertake a diagnostic of current assets, resources and plans and consider how far they can be leveraged for preventive care, using Health for Living’s key lines of enquiry and knowledge capital.
Develop an initial local Preventive Care vision and proposal for how its realisation should be governed.
Develop possible migration paths, including:
- actions that can be taken now, by individual providers or jointly
- changes that can be made to existing initiatives or plans to facilitate the provision of preventive care
- further development of service proposition and operating model for which investment might be needed and how evidence might be gathered to support the case
Identify any possible routes to external research or pump prime funding.
