The population is ageing quickly: more people living to acquire chronic conditions and frailty
The older population in England is going to grow substantially over the next 7 to 15 years.
The NHS and Local Authorities are not coping now, so will be overwhelmed if the model of care is not changed quite radically to emphasise prevention rather than cure.
